Who we can support
We support young people aged 16 to 25 who are leaving residential or foster care and are ready to begin living more independently with support.
For 16 and 17 year olds, support is arranged with the young person's social worker and reviewed through their care plan. For young adults aged 18 to 25, we work alongside their Personal Adviser and Pathway Plan.
Every referral is assessed individually. If we feel another service would better meet a young person's needs, we will tell you honestly and as early as possible.
Our staffing model
Day and night cover
There are day staff and waking night staff every day. Each shift may include Residential Support Workers, a Team Leader, a Deputy Manager or the Registered Manager, with one staff member coordinating the shift.
Allocated support
Every young person has an allocated staff member on each shift and a named key worker.
Management oversight
The Registered Manager, Dedecker Jacques Misambu, is the point of contact for each placement.
What happens after placement
We keep you informed and involved from the first day.
Moving in day
The young person is welcomed, shown around, given their keys and supported to settle into their room.
After 48 hours
A settling in meeting to check how the young person is doing and start agreeing independence goals.
First month
The support plan is developed with the young person's key worker, setting short and long term goals.
Every week
A 1:1 key work session, a planned weekly timetable and a progress summary.
Every month
The risk assessment and care plan are reviewed with the young person and shared with their social worker.
Reviews
Key workers write LAC review reports, which the young person contributes to.
Goals we work towards together
These are the goals we work towards with each young person. Every journey is different and progress is reviewed regularly.
